Ayurveda work published in PNAS

Dear All, As you know that we at IGIB, CSIR are working towards exploring genomic corelates of Ayurveda concept of prakriti and thereby identification of predictive markers for preventive and personalised medicine.We published our first paper on gene expression and biochemical correlates of extreme constitution types in 2008,in Journal of translational medicine, which you are aware of. Recently this group have been able to publish another paper in PNAS (Procedings of Bational Academy of Sciences), wherein we have been able to identify a gene and a genetic marker linked to high altitude adaptation and hypoxia responsiveness, through genetic analysis of extreme constitution types as defined in Ayurveda. While our paper was under review and later also ,3-4 new studies have reported the same gene in high altitude adaptation, albeit using diffrerent approaches. In most of these studies approach is that of High throughput/ Genome wide Association study whereas we could arrive at these results with very few SNPs. This not only validates our results about a gene but also established the role of Prakirit in identification of predictive markers. Modern way of understanding heavy metals

The prognostic classification in Ayurveda, curative, reversible with some difficulty, irreversible but can be controlled with medication, irreversible even cannot be controlled could not be changed about a single disease till date in spite of so much researches all over the world.

Some of the life saving & revolutionary molecules are driven from the herbs documented in Ayurveda then why not the time tested concepts, procedures, life-style at least till the time we reach in a better position thru researches in stead of doing some hit & trail experiments as in India; from my childhood I am keenly following these things e.g. An awareness campaign were carried out against feeding the child and now from years for feeding. We should also think about the radical control/cure on the line that a patient of hypertension carry on taking medicines for whole life even when there is no known related pathological factor, here I am talking about increase in dozes, numbers of medicines & their short /long –term side effects .Ok, right if this treatment can just control BP in terms of its related hazards on various systems of the body.

Ayurveda approach in DM-II

The term Madhumeha is used in Charaka Samhita [1000-1500BC], Susruta Samhita [100-500BC] for the multisystemic metabolic disorders effecting multitissues, Diabetes Mellitus. Incidences of Diabetes type-II, increasing along with the increase in materialistic orientation of society; the epidemic like situation is traced in the mutation of changes in related genes & metabolic mechanisms by scientists of the field; 230 millions of diabetics are living with disease.

It is emerging seventh leading causes of mortality. Increase in diabetes cases from 1990-1998 was 33% For people between 30-39 years the increase was 70% For people between 40-49 years the increase was 40% DM is caused either by a lack of Insulin [Type I] or body’s inability to use insulin [Type II] also known as maturity onset diabetes is often triggered by obesity stress& a sedentary life style.

In medical texts of Indian System of Medicine-Ayurveda Genetic susceptibility, indulgence in excess intake of milk & its products, sugarcane juice& its products, meat of animals from marshland, food products prepared from cereals that are less than one year, recently prepared wines, physical inactivity, excessive sleep, no day-to-day activity related mental plannings, excessive mental worry, obesity& sedentary life habits& faulty eating habits like eating of when earlier taken food is not digested, eating food without any appitite,overeating. CRF management in ayurveda

Chronic Renal Failure (CRF) is the progressive loss of kidney function. The kidneys attempt to compensate for renal damage by hyperfiltration (excessive straining of the blood). Over time, hyperfiltration causes further loss of function. Chronic loss of function causes generalized wasting (shrinking in size) and progressive scarring within all parts of the kidneys. In time, overall scarring obscures the site of the initial damage. Yet, it is not until over 70% of the normal combined function of both kidneys is lost that most patients begin to experience symptoms of kidney failure. Signs and Symptoms of Chronic Renal Failure Chronic renal failure (CRF) usually produces symptoms when renal function — which is measured as the glomerular filtration rate (GFR) — falls below 30 milliliters per minute (< 30 ml/min). This is approximately 30% of the normal value. When the glomerular filtration rate (GFR) slows to below 30 ml/min, signs of uremia (high blood level of protein by-products, such as urea & creatinine) may become noticeable. When the GFR falls below 15 ml/min most people become increasingly symptomatic. Uremic symptoms can affect every organ system in the body. Chronic renal failure nutrition should have a balance of fluids and electrolyte and adjusting the diet to prevent accumulation of toxic waste product to the minimum possible level. Chronic renal failure nutritional needs are as follows: The food should be high in calories and low in protein. Avoid canned, baked, processed and preserved foods. Salted cheese, butter and nuts should be avoided To reduce potassium content, vegetables should be cooked with more water and excess should be drained off. Avoid fruits, fruit juices, and coconut water. If potassium is restricted avoid tea, coffee, bajra, maize, raji, wheat flour, pulses and legumes, chocolates, vegetables, fruits, sauces, jam, jellies. Treatment for renal failure Prevention is always the goal with kidney failure. Chronic disease such as hypertension and diabetes are devastating because of the damage that they can do to kidneys and other organs. Lifelong diligence is important in keeping blood sugar and blood pressure within normal limits. Specific treatments are dependent upon the underlying diseases. Once kidney failure is present, the goal is to prevent further deterioration of renal function. If ignored, the kidneys will progress to complete failure, but if underlying illnesses are addressed and treated aggressively, kidney function can be preserved, though not always improved. From Ayurveda Philosphical language to molecular biology

A landmark study, the first of its kind in the world, finds links between Prakriti, a fundamental principle of personalized medicine of Ayurveda, and modern genomics for development of predictive and personalized medicine. The study reveals that it is possible to identify groups within normal individuals of the populations, which could be predisposed to certain kind of diseases, and also might respond differently to drugs. Such integration of the principles of Ayurveda with genomics, appropriately termed as Ayurgenomics by the authors, holds great potential and promise for future predictive and personalized medicine at an affordable cost. For the first time it has been demonstrated that normal individuals within the same ethnic population, clustered on the basis of clinical criteria described in Ayurveda, show variations in the basal levels of blood parameters used in routine for diagnostic purposes, as well as in basal levels of expression of genes. Human genome sequencing and subsequent mapping of genetic variability has allowed us to identify genetic variations and patterns of variability across diverse global populations. India has also put systematic and concerted efforts to identify relatedness amongst the diverse Indian populations at genetic level. The first step in this regard was the CSIR led Indian Genome Variation Consortium project which provided the first genetic landscape of India. In this project, 55 distinct populations of India, classified on the basis of linguistic lineage and ethnicity from different geographical zones of the country, were found to cluster into five major groups of relatedness. However, despite greater genetic similarity there was high inter-individual variability within each cluster. This inter-individual variability confers differential susceptibility to common diseases like diabetes, asthma, cardiovascular disease and differential drug responsiveness. Therefore, the next logical step was to identify groups within these related populations that are likely to be similarly predisposed or protected from the disease. According to Ayurveda, an individual’s basic constitution, Prakriti, to a large extent, determines predisposition and prognosis to diseases as well as suitability of preventive and curative regimen for the same. This study explored whether this ancient and documented system of Prakriti analysis could be correlated in modern biology terms. For this purpose, more than 1000 individuals were screened, out of which 120 individuals of predominant prakriti were identified, and subsequently 96 unrelated ethnically matched healthy individuals with predominance of either Vata (39 individuals), Pitta (29) or Kapha (28) prakriti, belonging to an age group of 18 – 40 years with equal numbers of both genders (n=48 in each case) were recruited for further analysis. Blood sampling was carried out from these individuals following all ethical guidelines. DNA, RNA and serum were isolated and whole genome expression profiling, using H19Kv8 cDNA microarray as well as biochemical testing for 33 parameters used in routine testing was carried out to study whether these groups exhibit any differences. These subjects were all from North India and of Indo-European origin. The genetic background of the subjects was reconfirmed through analysis of genetic similarity with this group and populations of the Indian genome variation project. The subjects shared genetic affinity with the Indo-European populations of North India. In this study, normal individuals of the three most contrasting Prakriti types, namely, Vata, Pitta and Kapha, were identified following clinical criteria described in Ayurveda in Indian population of Indo-European origin. It was observed that these Prakriti types exhibited differences at biochemical profiles like liver function tests and lipid profiles and hematological parameters like hemoglobin level etc. Differential gene expression was found in significant number of housekeeping and disease related genes. A significant variation in expression of genes related to metabolism, transport, immune response and regulation of blood coagulation etc. was also observed. The results of the study conceptualized by Prof Samir Brahmachari, Director General, CSIR and led by Dr. Mitali Mukerji at the Institute of Genomics and Integrative Biology have been reported in the Journal of Translational Medicine recently (September 2008). There was intense involvement of Dr. Bhavana Prasher an Ayurveda expert and Dr. Sapna Negi a senior molecular biologist (senior authors of paper) along with a team of trans-disciplinary people that included Ayurveda doctors, molecular biologists, biochemists, bioinformaticians and statisticians. In view of Dr. Ramniwas Prasher; Ayurgenomics can be foundation stone laid by Dr. S.K Brahmachari DG ;CSIR & his team of scientists including Dr. Bhavana Prasher as an Ayurveda expert, not only in curative but also in preventive medicines once this mission of co-relation between the Ayurvedic genetic -physiologial factors VATA-PITTA & KAPHA modern molecules of genetics could be traced as Stratified lifestyle & dietary-regimen & guidelines can be in accordance to the types of Prakriti that will check the manifestation of the diseases in spite of having proneness to that categories of diseases

Cancer management in ayurveda

The incidences of various types of cancers are increasing so much that The Cancer war was declared in 1971 by cancer industry & Federal govt.Since then 2trillian USD on conventional treatment but success is just 2.4%.In this system the chemotherapeutic agents mostly derived from herbs..but the artificially formed these active agents produce much more side effects. Ayurveda have better option in improving the quality of life,reversing the disease process involved & by improving the immunity level against carcinogens.
After a vigorous study on the subject from ayurveda dr.prasher developed his insight in this particular subject & treated a few cases of different types of cancers & noted that Ayurveda system of treatment helpful in overcoming this dreaded disease at various stages-
